Independent living home that offers transitional housing and long term clean and sober living environment. Office Location: 4358 North Foster Drive Baton Rouge, Louisiana 70805 They serve individuals who suffer from a variety of substance abuse and mental health issues, seeking an alcohol and drug free environment for long and short term housing. Residents are able to live in an individual room or a shared room. Admission Criteria: A.Referral from any behavioral or mental health agency B.Ability to live independently including cooking and cleaning C.Documented disability D.Inadequate social, community and/or family support system E.Proof of income F.Ability to pay room rental and one time processing fee G.Ability to comply with all conduct and house rules H.Ability and willingness to participate as prescribed by treating/referring physician in daily treatment plans I.Completed referral package including a.List of Medication b.FACE sheet including any diagnosis c. Ability to pass a drug test Services: Comfortable, safe and engaging home Fully furnished units with kitchen appliances, utensils, and all cookware All utilities included Washer/Dryer Cable Exercise equipment Grocery items Computer and internet access with printing capabilities Convenient bus line route Individual room keys and storage lockers Counseling and day programs provided by various local behavioral health care sites
