The Grace House community operates on a phase system and is supervised 24/7/365. The community functions under a therapeutic residence model and case management services. While living at Grace House, residents are required to develop and progress through an Individual Service Plan. This Individual Service Plan will include participation in outpatient substance abuse treatment, participation in community based 12-step meetings on a daily basis, and full participation in all activities of the Grace House community.
