Lakes and Prairies provides supportive services to homeless families/individuals who meet the eligibility criteria, have a strong desire to break the cycle of homelessness and are willing to work on specific goals. These programs focus on families and individuals who typically have a difficult time reaching self-sufficiency due to one or more of the following issues: mental illness, domestic violence, chemical dependency, unlimited independent living skills, and/or lack of marketable skills/education. Clients are provided with the support and guidance necessary to maintain stable housing.  Services are based on the family or individual specific needs. They include but are not limited to: * Case Management. * Referrals (Employment, Health, Food, Clothing, Social Services...). * Education. * Advocacy. * Financial assistance. * Transportation

On probation out of hennipen co. Seeking sober housing or outpatient treatment by family in Barnesville. Rule 25 completed. Recently discharged self from thrive treatment in Alexandria. Mental health wasn't addressed and housing situation there was unhealthy. Been sober sense 11/18. Still in need of sober network and mental health help.
